Nurse Associate – Full-Time | Brighton

Hours: Full-time – 37.5 hours per week

We are seeking a compassionate and proactive Nurse Associate to join a PCN on a full-time, fixed-term basis.
This is an excellent opportunity to support the delivery of proactive, community-based care for vulnerable and housebound patients, helping them to maintain their health and independence within their own homes.
Key Responsibilities:
• Undertake planned home visits to vulnerable and housebound patients.
• Complete clinical observations, health checks and delegated clinical tasks within your scope of practice.
• Support the monitoring and management of long-term conditions.
• Recognise and escalate any changes in a patient’s health, wellbeing or clinical presentation.
• Provide timely and accurate feedback to the nursing team following patient visits.
• Contribute to a proactive, preventative approach to patient care, identifying unmet needs and supporting patients to remain well at home.
• Maintain accurate patient records using EMIS and SystmOne.
Requirements:
• Registered Nurse Associate.
• Experience working within Primary Care, Community Nursing or a similar setting is essential.
• Familiarity with EMIS and/or SystmOne is essential.
• Full UK driving licence and access to a vehicle are essential due to the community-based nature of the role.
