About the Role

We are currently recruiting a Chronic Disease Practice Nurse for a GP practice in Emergency Emergency Caernarfon. The role suits a nurse with primary care experience who wants variety across chronic disease clinics, immunisations and treatment room work.

Key Responsibilities

• Run chronic disease clinics including asthma, COPD, diabetes and hypertension.
• Deliver childhood and adult immunisations and travel vaccinations.
• Carry out cervical screening and wound care.
• Provide health promotion and lifestyle advice.
• Maintain accurate records on the practice clinical system.

Requirements

• Registered Nurse with current NMC PIN
• Primary care or treatment room experience
• Confident with cytology, immunisations and long term condition reviews
• Able to work independently and as part of a team
