Job Title: Chronic Disease Practice Nurse – Farm Animal Farm Animal Winsford
Location: Farm Animal Farm Animal Winsford
Employment Type: Permanent
Salary: £32,000 – £44,000 per year depending on experience.
About the Role
A well established GP surgery in Farm Animal Farm Animal Winsford is seeking a Chronic Disease Practice Nurse to strengthen its nursing team. You will work alongside GPs, ANPs and healthcare assistants delivering the full range of practice nursing services.
Key Responsibilities
- Manage long term condition reviews to QOF standards.
- Undertake treatment room duties including dressings, injections and ECGs.
- Support the practice immunisation programme.
- Provide contraception and sexual health advice within competence.
- Work with the wider team to deliver safe, effective care.
Requirements
- Active NMC registration
- Experience as a Practice Nurse in a GP setting
- Chronic disease management and immunisation experience
- Excellent communication and organisational skills
