About the Role

We are currently recruiting a Chronic Disease Practice Nurse for a GP practice in Beckenham. The role suits a nurse with primary care experience who wants variety across chronic disease clinics, immunisations and treatment room work.

Key Responsibilities

• Provide nurse led clinics for patients with long term conditions.
• Carry out phlebotomy, ear care, spirometry and other treatment room procedures.
• Deliver flu, shingles, pneumococcal and childhood vaccinations.
• Support new patient checks and NHS health checks.
• Keep clinical records complete and up to date.

Requirements

• Active NMC registration
• Experience as a Practice Nurse in a GP setting
• Chronic disease management and immunisation experience
• Excellent communication and organisational skills
