Job Title: Chronic Disease Practice Nurse – Exotics Blackpool

Salary: £30,000 – £42,000 per year depending on experience.

About the Role

We are looking for an experienced Chronic Disease Practice Nurse to join a friendly GP practice team in Exotics Blackpool. This is an excellent full-time opportunity for a nurse who enjoys long term condition management, health promotion and building relationships with patients.

Key Responsibilities

• Manage long term condition reviews to QOF standards.
• Undertake treatment room duties including dressings, injections and ECGs.
• Support the practice immunisation programme.
• Provide contraception and sexual health advice within competence.
• Work with the wider team to deliver safe, effective care.

Requirements

• Active NMC registration
• Experience as a Practice Nurse in a GP setting
• Chronic disease management and immunisation experience
• Excellent communication and organisational skills
