Job Title: Chronic Disease Practice Nurse – Emergency Hitchin North
Location: Emergency Hitchin North
Employment Type: Permanent
Salary: £32,000 – £44,000 per year depending on experience.
About the Role
We are looking for an experienced Chronic Disease Practice Nurse to join a friendly GP practice team in Emergency Hitchin North. This is an excellent permanent opportunity for a nurse who enjoys long term condition management, health promotion and building relationships with patients.
Key Responsibilities
- Run chronic disease clinics including asthma, COPD, diabetes and hypertension.
- Deliver childhood and adult immunisations and travel vaccinations.
- Carry out cervical screening and wound care.
- Provide health promotion and lifestyle advice.
- Maintain accurate records on the practice clinical system.
Requirements
- Registered Nurse with current NMC PIN
- Primary care or treatment room experience
- Confident with cytology, immunisations and long term condition reviews
- Able to work independently and as part of a team
