An opportunity has become available for a Chronic Disease Practice Nurse in Ipswich. The practice is forward thinking, well organised and puts patient care and staff wellbeing first.

Key Responsibilities

• Run chronic disease clinics including asthma, COPD, diabetes and hypertension.
• Deliver childhood and adult immunisations and travel vaccinations.
• Carry out cervical screening and wound care.
• Provide health promotion and lifestyle advice.
• Maintain accurate records on the practice clinical system.

Requirements

• Active NMC registration
• Experience as a Practice Nurse in a GP setting
• Chronic disease management and immunisation experience
• Excellent communication and organisational skills
