Job Title: Chronic Disease Practice Nurse – Emergency Clitheroe
Location: Emergency Clitheroe
Employment Type: Part-Time flexible sessions
Salary: £30,000 – £42,000 per year pro rata depending on experience.
About the Role
We are currently recruiting a Chronic Disease Practice Nurse for a GP practice in Emergency Clitheroe. The role suits a nurse with primary care experience who wants variety across chronic disease clinics, immunisations and treatment room work.
Key Responsibilities
- Manage long term condition reviews to QOF standards.
- Undertake treatment room duties including dressings, injections and ECGs.
- Support the practice immunisation programme.
- Provide contraception and sexual health advice within competence.
- Work with the wider team to deliver safe, effective care.
Requirements
- Registered Nurse with current NMC PIN
- Primary care or treatment room experience
- Confident with cytology, immunisations and long term condition reviews
- Able to work independently and as part of a team
