Job Title: Chronic Disease Practice Nurse – Exotics Swaffham
Location: Exotics Swaffham
Employment Type: Part-Time flexible sessions
Salary: £30,000 – £42,000 per year pro rata depending on experience.
About the Role
We are looking for an experienced Chronic Disease Practice Nurse to join a friendly GP practice team in Exotics Swaffham. This is an excellent part-time opportunity for a nurse who enjoys long term condition management, health promotion and building relationships with patients.
Key Responsibilities
- Run chronic disease clinics including asthma, COPD, diabetes and hypertension.
- Deliver childhood and adult immunisations and travel vaccinations.
- Carry out cervical screening and wound care.
- Provide health promotion and lifestyle advice.
- Maintain accurate records on the practice clinical system.
Requirements
- Active NMC registration
- Experience as a Practice Nurse in a GP setting
- Chronic disease management and immunisation experience
- Excellent communication and organisational skills
