Job Title: Chronic Disease Practice Nurse – Burnham-on-crouch
Location: Burnham-on-crouch
Employment Type: Part-Time flexible sessions
Salary: £30,000 – £42,000 per year pro rata depending on experience.
About the Role
We are looking for an experienced Chronic Disease Practice Nurse to join a friendly GP practice team in Burnham-on-crouch. This is an excellent part-time opportunity for a nurse who enjoys long term condition management, health promotion and building relationships with patients.
Key Responsibilities
- Manage long term condition reviews to QOF standards.
- Undertake treatment room duties including dressings, injections and ECGs.
- Support the practice immunisation programme.
- Provide contraception and sexual health advice within competence.
- Work with the wider team to deliver safe, effective care.
Requirements
- Active NMC registration
- Experience as a Practice Nurse in a GP setting
- Chronic disease management and immunisation experience
- Excellent communication and organisational skills
