Job Title: Senior Practice Nurse – Todmorden

Employment Type: Part-Time flexible sessions
Salary: £30,000 – £42,000 per year pro rata depending on experience.

About the Role

We are looking for an experienced Senior Practice Nurse to join a friendly GP practice team in Todmorden. This is an excellent part-time opportunity for a nurse who enjoys long term condition management, health promotion and building relationships with patients.

Key Responsibilities

• Run chronic disease clinics including asthma, COPD, diabetes and hypertension.
• Deliver childhood and adult immunisations and travel vaccinations.
• Carry out cervical screening and wound care.
• Provide health promotion and lifestyle advice.
• Maintain accurate records on the practice clinical system.

Requirements

• Active NMC registration
• Experience as a Practice Nurse in a GP setting
• Chronic disease management and immunisation experience
• Excellent communication and organisational skills
